Services Overview


Home Care Services

Personalized care up to 24 hours a day by experienced and registered home care aides. All Lifespan home care aides are trained, bonded, insured, undergo an extensive background check, and are supervised by aging care professionals.

Support with personal care, meal preparation, and light housekeeping.

Calm, confident, and compassionate care for individuals with dementia-related conditions.


Coordination with hospital discharge planners to ease transition and speed recovery.


Coordination with local hospice organizations to provide enhanced support to families caring for loved ones at the end of life.


Partnership with the VA to provide respite services to qualifying veterans.

Care Management Services

A registered nurse or social worker works with clients, their family members, and medical professionals to assess needs and develop a plan of care.


A plan of care is recommended and implemented by the Lifespan team to promote the highest level of health, safety, comfort and independence for that client.


Help with set-up and refill of pill organizers.


Evaluation of health and psycho-social needs and abilities, home safety, available resources, and client goals.


Regular visits to monitor status and supervise care. Care coordination with medical care team and identification of potential problems to prevent crises.


Assistance to locate and arrange the best match for out of home care when necessary.

Additional Support Services



Connecting clients with a skilled assistant for life enhancement activities such as socialization, recreation, home organization, and more, including medical appointment escorts.


Telephone consultation and counseling regarding aging issues by an RN.
